I swam across a river and traveled home (Teleported) after I was sure I had moved far enough away to be out of combat. (The combat icon in the top right was completely gone.) My black bile stopped draining after I teleported and the Fear debuff ran out. A minute or two later My black bile suddenly dropped from 15 to 0 and knocked me out. I am sure it instantly dropped as I was looking at it for food I was about to eat. I am using Ender's Client if that is relevant.
